We are an innovative forward thinking practice that likes to challenge perceptions of general practice. We are growing the multi-disciplinary team at Hope Farm Medical Centre and in addition to the traditional skill mix of doctors, advanced nurse practitioner, practice nurses and healthcare assistants at the practice, we now also have within our skill mix physiotherapists, wellbeing coordinators and social prescribing link workers, a clinical pharmacist, GP Assistants, Care Coordinators and an Occupational Therapist. This enables the medical centre to provide a holistic approach to our patient population, leading the development of these innovative roles within the general practice workforce.
We also score highly by patients in surveys and feedback, and at our last inspection we were graded as outstanding by the CQC for the caring domain. An exciting opportunity has emerged for a Practice Operations Manager with real passion and enthusiasm for managing and leading a team within a forward-thinking and progressive medical centre. They will work closely with the Practice Operations Manager (Facilities Planning and Management), Practice Operations Manager (IT and Data Quality) and the Managing Associate Partner as part of the senior management team at Hope Farm Medical Centre. With a high degree of organisational skills, you will be empowered to ensure the Patient Services Team are managed and delivering on a day-to-day basis. We are a friendly and supportive GP practice, delivering high levels of patient care, which prides itself on our positive patient feedback. As one of the practice ambassadors we are seeking an outgoing person with a can-do friendly attitude to join our Patient Services Team. You will be responsible for undertaking a wide range of reception, administrative and care navigation duties and the provision of general support to the multidisciplinary team at Hope Farm Medical Centre. The Patient Services Team forms an important part of the operational team and work under the direction of the Operations Manager Practice & Patient Services Team. Specific duties can include but are not limited to, greeting and directing patients, patient registration, booking appointments, processing of information (electronic and hard copy) and assisting patients as required. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.
